| Shady Creek Farm & Puppy Rescue Puppy or Dog Adoption Contract I, ____________________ agree to sell a ______________________ puppy male or female called _______________, as a pet to ______________________ on the date of ______________, 2005 for $_______.00. It has already had it's _______ shots and was worming ___________, 2005. It also has had __________________ on _____________. The buyer agrees to get the dog all required shots, heart worm preventive medication, rabies shots and keep updated for the life of the dog, to keep it healthy. If you do not wish to keep it, you may return it with in 2 weeks for a full refund. I cannot guarantee against any fleas, worms, ticks, allergies, skin irritations, intestinal parasites or other parasites, although I try to prevent any of these. If at anytime during the life of the dog the buyer does not feel they can or want to keep the dog for any reason they must return the dog to me or contact me if the dog ‘s ownership is to be changed. I will come and get the dog if you can no longer care for the dog. If I can resell or place the dog you will receive any money, if any, that I get from the resale at that time. This dog is sold as a ______ ___________________ breed with ____ papers. Birth date is ______________, 200_. Mom is a ___________________. The buyer of a rescue puppy agrees to spay or neuter this dog before it is 12 months old. Not necessary for AKC pure bred registered dogs. I do hope this puppy is a treasured member of your family for many years. I do hope all of my dogs are healthy and live long and bring much joy to their new family, as my dogs have enriched my life greatly. I will not be responsible for ANY VET BILLS the buyer has for this puppy. It is healthy as far as I have seen. I will give the puppy a bath and dip the morning of leaving here to go to his new forever home. Buyer agrees to pay for the shipping expenses before the puppy is sent or picked-up. A $50.00 deposit is non- refundable if you change your mind. If a deposit is put on a puppy $50.00 per puppy of it is
non-refundable, if you change your mind. The money has usually
been spent on dog food to feed all the rescues we have here. The
purpose of a deposit is to show you do intend to buy the dog so it should
be non-refundable unless you send over $50.00 per dog or puppy.
